Intake Forms is the custom form builder in My Workspace. Create guest cards, event check-in forms, therapy client intake sheets, or fully custom forms — then publish a standalone page, embed it anywhere, and review every submission in one inbox. Available on every plan at no extra cost.
Intake Forms does not replace Volunteer Applications. That queue stays in the People hub under People → Applications and keeps its own approval workflow.
Forms and Submissions
The Intake Forms shell has two tabs: Forms and Submissions. Forms lists every intake form your organization has built. Submissions is the shared inbox for all captured responses across all active forms.
Add New Form
Click Add New to open the template chooser. Wave-1 templates cover the most common cases:
- Church visitor — captures guest identity and writes a Database person record; AlignMint matches to an existing record when identity is clear and flags ambiguous submissions for staff review
- Event check-in — records attendance as a submission without a People write
- Therapy client intake — captures intake answers as a submission without a People write
- Customize your own — blank canvas for any other use case
After choosing a starting point, name the form, configure questions and branding, then save as a draft or publish immediately.
Publish and Share
Every intake form gets a public URL and a branded QR code you can download for print or digital use. You can also embed the form on any external site using the embed snippet from the form settings panel. Both the standalone page and the embed respond to the same submission path — responses always land in the same inbox.
Published forms stay at a stable URL. Archiving a form takes the public page offline without deleting submissions.
Submissions Inbox
The Submissions tab shows every response across all your intake forms in one place. Each row shows the form name, submission timestamp, and a preview of the captured answers. Open a row to see the full response. Church visitor submissions that matched or created a Database record show a link to that person's profile.
Every successful submission is stored as an immutable record — responses cannot be edited after capture.
Assignee Notifications
Each form can list up to 25 staff assignees. When a new submission arrives, AlignMint sends a bell notification to every active assignee on the form. If no assignees are set, no notification fires. Assignees are set per form in the form settings panel, not per submission.
Volunteer application submissions use a separate notification path in Volunteer Settings and are not affected by intake assignees.

- Review all submissions in the Submissions inbox across all forms
- Assign staff members to receive bell notifications on new submissions
- Archive a form to take it offline without losing submission history
Good to Know
Intake Forms is available on every plan — the same tier access as Database. Write paths follow existing People and volunteer director permission rules per form flavor.
Volunteer Applications are not part of Intake Forms. The application review queue, approval workflow, and email settings live in People → Applications (Volunteer hub). See Volunteer Applications.
Public Scheduling is a separate tool for booking-page availability — it is not the same as an intake form.
